Prescriber's Guidelines - (con't)

These guidelines were developed for the use by Stony Brook University Hospital’s Acute Pain Service.

                                Converting to Oral Analgesics

IV MSO4 1- 1.5mg = about 5mg Oral oxycodone
If requirements > 30mg/24 hours
:
OxyContin 10mg q 12 hours at 10am and 10pm
With Roxicodone 5mg q 4 hours PRN mild breakthrough pain, 10mg PO q4 hours PRN moderate-severe pain
If requirements > 60mg/24 hours: Keep PCA for breakthrough pain x 24hrs and start OxyContin 20mg q 12 hours at 10am and 10pm.
Evaluate need to increase OxyContin and dose roxicodone.
If requirements > 90 mg/24 hours: Keep PCA for breakthrough pain x 24hrs. And start OxyContin 30 mg q 12 hours at 10am and 10pm. Evaluate dosing of roxicodone.
Discuss with attending or nurse practitioner patients with higher requirements.
If requirements exceed 100mg/24 hours: Consider keeping PCA
