Prescriber's Guidelines - (con't)

These guidelines were developed for the use by Stony Brook University Hospital’s Acute Pain Service.

                                                Second Line

DILAUDID: 1mg/cc (custom made by pharmacy)

Useful in patients who have a non-allergic morphine intolerance (nausea/vomiting) or end-stage renal disease

FOR OPIATE NAÏVE
CONTINUOUS: None
DEMAND: 0.3mg-0.6mg
LOCKOUT: 8 minutes
4-HOUR MAXIMUM (base on 4-5 demands/hour + continuous)
Loading Hydromorphone 1mg=5-7mg Morphine. Once loaded
Hydromorphone 1mg = 3mg Morphine
