These guidelines were developed for the use by Stony Brook University
Hospital’s Acute Pain Service.
THORACIC
EPIDURAL: Elderly/Infirm (or low baseline B/P)
Bupivacaine 0.05% with Hydromorphone PF 10 micrograms/cc in 200cc PF Normal
Saline
(Patients > 80 yrs old or pts. very responsive to opiates—Use
5 micrograms/cc of Hydromorphone PF)
Settings are the same as regular strength Epidural
OPIOID TOLERANT patients to be discussed with the
Acute Pain Attending.
